Updating your details

If you change name, address or telephone number, please let our receptionist staff know by either telephoning, calling in to our reception desk, e-mailing your changes or putting in writing to us stating who has changed address and your new address details along with your postcode and any new telephone number. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card or you will be informed by a member of our staff to find a new doctor within 4 weeks of moving out of our catchment area.
